In an effort to provide our patients with fast convenient access to their films, we have made the process simple and easy.

After downloading the forms, please call 732-292-3158 or 732-292-9980 and ask for Val. She will answer any questions you may have regarding the forms. When the form is completed is complete, please fax it to 732-292-9950.

For your convenience, if we have the form at least 2 weeks in advance, we will pick up your films for you. Otherwise, please contact your radiology provider and ask them how you can obtain your films.
